We need the following information from you no later than Wednesday, January 30:

• Number of teams in each division
  Elementary Division – Kindergarten – 4th grade (officials are not scheduled for this division, but we do need to schedule the games)
  Middle School Red Division – primarily 7th – 8th grade
  Middle School Blue Division – primarily 5th – 6th grade
  Middle School Purple Division – mix of 6th – 8th grade (this is for teams that only have one team and have a fairly even distribution across ages)
• Any conflicts you already know that will prevent you from playing the following dates:
  March 23 & 24, April 6 & 7, 13 & 14, 20 & 21, 27 & 28
  We have had a request to possibly schedule games on Friday – that most likely will NOT happen
• Your field availability (we cannot schedule without this information)
  Date, Time and Location (with field number if applicable)

• Contact Info for your team
  Team Name, Contact Person, Address, Phone Number
